Prior to Your Visit:
For your convenience we have included our new patient registration form here on our website. Please complete this form prior to your appointment. Bring this form along with your dental insurance card (if applicable). Please arrive a few minutes early for your appointment. All patients under 18 must be accompanied by a parent or guardian.
Day of Your Visit:
Quality dental care requires a team effort. Your initial visit will be the foundation to a long term and trusting relationship with your dentist. Our staff will address any concerns and expectations you have and ensure you experience a comfortable visit.
Your clinical exam will begin with digital x-rays which instantaneously produce high resolution images of your teeth. A complete and through exam will be completed by Dr. Clark. This involves a screening for oral cancer, tooth-by-tooth charting to identify and record the current condition of your teeth, and a periodontal exam to assess the condition of your gum tissue. An intraoral camera may be used to show you the condition of your teeth. Dr. Clark will provide you with a dental report and treatment plan. He will discuss any necessary procedures that are required to address your dental health needs as well as any alternatives or options. Often your first visit will include a comprehensive general cleaning, though if it has been awhile since your last cleaning, you may be scheduled for a longer visit at a later time.
We accept most major insurance plans and will gladly process your claim. Insurance policies generally cover only a portion of the total treatment cost. Please remember that your dental insurance policy is an agreement between you and your insurance company, not between your insurance company and our office. We make no guarantee of any estimated coverage but we will do our best to ensure that you receive your maximum benefits. Please keep in mind that you are responsible for your total obligation of fees for the treatment provided should your insurance benefits result in less coverage than anticipated. If you would like to know what your expected coverage will be, we can submit a pre-determination estimate to your insurance company.
Unless other arrangements have been made, we ask that you pay your portion of the bill at the time of treatment. It is your responsibility to pay any balance not paid by your insurance company.
